How To Take Care Of Your Skin at 20, 30, 40 and 50


No matter your age, your skin needs love, nurturing and care. In order to fend off aging for as long as possible, it’s crucial to start caring for your skin as early as you can. At each point in life Dr. Melamed recommends following the 4 Commandments of Skincare:

  1. Cleans your skin twice a day: use an appropriate AHA, BHA, or gentle cleanser.
  2. Exfoliate your skin in evening: options include various retinoic acids, manual exfoliation, or mechanical exfoliate. The method and frequency will depend on your age and skin concerns.
  3. Moisturize your skin twice a day: use a non-emollient moisturizer to keep your skin hydrated. This is a great opportunity to include special vitamins, antioxidants, and compounds to manage excess pigmentation. Dr. Melamed can create custom compounds that meet your specific needs.
  4. Protect your skin every morning: use a minimum SPF 30 to protect from UVA and UVB radiation. Make sure to reapply sunscreen as the effectiveness will diminish after 2 hours of sun exposure.

Skin care does not have to include invasive procedures or expensive creams– in fact, each decade will require a slightly different approach.

In your 20’s…

Whilst your skin is plump and full of collagen, it’s time to start preserving it. One of the most common causes of aging and sagging is sun damage – so be sure to protect yourself!

  • Always cleanse thoroughly and remove make up before bed. Try not to wear excessive or heavy make-up and be gentle on your skin when you remove it.
  • You’re never too young to moisturize. Creams containing anti-aging ingredients are a great deterrent, but be sure to moisturize daily in some way.
  • Use an SPF wherever possible – in your moisturizer and foundation. Stay out of the sun if you can.
  • If you’re partying, be sure to drink lots of water and treat your skin to a hydrating face mask once or twice a week.

In your 30’s…

As smaller lines, discoloration and wrinkles start to set in, you might want to think about slightly thicker and stronger treatments or creams.

  • Add serums into your routine. For an extra boost, choose ones which contain vitamin C or Retinol.
  • Look into low level acids like hyaluronic acid and lactic acid. These can be found in or added to your daily moisturizer.
  • Cleanse morning and night. Try oily or creamy cleansers and try not to drag on the skin, especially around the eye area.
  • Moisturize vigilantly and stay out of the sun. Avoid things that will dehydrate the skin, such as excessive alcohol and coffee.

In your 40’s…

Years of sun exposure, make-up and pollution may start to show itself in your skin. Targeted treatments may produce more effective results at this stage.

  • Eye creams with peptides and anti-aging ingredients can be used at night. Wear sunglasses when outside to prevent squinting.
  • Differentiate between day and night skincare. Use serums, oils and peptides at night. Exfoliate, cleanse and lightly moisturize during the day.

Medical treatments such as TCA peels, microdermabrasion and collagen stimulators can all deliver long-lasting results through non-invasive procedures.

In your 50’s…

Skin becomes drier and the loss of elasticity causes deeper wrinkles. These lines may be most evident around the mouth, eyes and neck. Lower oestrogen levels can cause skin dehydration from the menopause, onwards.

How To Treat Rosacea: From Tablets To Treatments

Rosacea is a fairly common chronic condition, affecting almost 16 million American adults. [1] Facial redness, occasional pimples, tiny red veins and stingy skin are some of the signs of rosacea. It is often worsened by alcohol, heat, exercise and sun exposure. Whilst it can be covered with make-up or diminished with creams, many people search for a more permanent and long lasting solution to their skin-reddening issue.

There are 3 main phases of rosacea:

  • Pre-rosacea is when the skin appears flushed due to dilation of blood vessels in the skin.
  • Vascular rosacea is when the symptoms worsen and skin begins to redden. There may be some minor inflammation.
  • Inflammation is when the skin becomes bumpy and blistered, particularly on the nose, cheeks, forehead and chin.

This chronic condition cannot be cured, but it can be significantly improved with the right treatment.

Lifestyle changes

Many rosacea sufferers find that particular foods or activities tend to worsen their symptoms. For example, sun exposure, vigorous exercise and thick make-up may exacerbate the issue. Wear an SPF daily and keep your skin out of the sun where possible. Keeping hydrated and taking good care of your skin is the easiest and simplest way to keep mild rosacea under control. Avoid spicy foods and caffeinated hot drinks, especially in hot weather.

Antibiotics

For sudden or stubborn flare-ups, antibiotic gels and medications are available. These include ingredients such as metronidazole, azelaic acid and ivermectin. These topical treatments are short-term and should improve symptoms fairly quickly. They can also be used on an ad-hoc basis in many cases but must be prescribed to you by a professional.

Isotretinoin (Accutane)

Isotretinoin capsules are often used to treat acne and spots. However, the capsules and gel can be used to treat mild forms of rosacea. You may need to use specialist moisturizers to protect your skin as isotretinoin can be quite drying.

Tretinoin Cream (Retin-A)

Retin-A and Tretinoin are forms of retinoids – retinoids encourage and affect the growth of skin cells. Ongoing use of retinoid products can reduce redness, pimples and skin irregularities. In some cases, however, retinoids may worsen redness.

IPL Treatment

IPL (or Intense Pulsed Light) uses lasers to reduce the number and size of dilated blood vessels in the face, as well as flushing, redness and breakouts. After only one or two treatments, symptoms can be greatly reduced. This treatment will simultaneously reduce brown spots and pigmentation caused by sun exposure and againing. Results may require 3-4 treatment sessions spaced 3 weeks apart.

Radiofrequency Vein Treatment

Radiofrequency is a colour-blind system that uses energy to eliminate the small unwanted vessels seen in vascular rosacea. The red vessels that accumulate on the nose, cheeks, and chin can be effectively eliminated. Results are immediate and may require an additional follow-up after 2-3 months.

How Hormones Might Be Aging You & How to Combat Them


Hormones control almost every aspect of our bodies. As we get older, we start to lose hormones and our ability to reproduce them, this can cause hormonal imbalances and a whole string of weird and unexpected symptoms.

Our skin is particularly sensitive to fluctuations in hormones – with three hormones causing most issues. Depending on the hormone at fault and the symptoms, there are a variety of different treatments to investigate.

Estrogen

Sagging skin and poor hydration is usually caused by this hormone. Elastin and collagen production decreases when there is an estrogen dip – resulting in fines lines, drooping around the eyes and mouth and a general loss of firmness. Low-estrogen often occurs in post-menopausal women, among other causes.

  • Treatment:

Hormone Replacement Therapy (HRT) is always an option to discuss with Dr. Melamed. However not all women are ready or are good candidates for HRT.

An alternative option is to address the symptoms by using fillers to add back volume and to stimulate elasticity. Fillers such as Versa, Restylane, Belotero, and Juvederm are highly effective collagen-like products that rejuvenate the skin, restore elasticity and deliver a plump and glowing complexion. These safe and natural fillers take just 30 minutes to deliver and can last 8-24 months (depending on the area treated and product used). Results are immediately apparent.

Testosterone

  • Acne Treatment:

Laser treatments, Photodynamic Therapy (PDT), and various medical remedies are extremely popular for treating adult acne. PDT uses blue and red light treatments in combination with alpha levulenic acid (ALA) to reduce excess oiliness and bacterial growth. Thyroid

If your thyroid gland is under or over active, you will notice changes in your skin and hair texture. Dry, rough, and pale skin are often signs of an under active thyroid. As the skin dries, it becomes more prone to wrinkling, sagging and general aging.

  • Treatment:

After a complete hormone assessment, and appropriate supplementation, Dr. Melamed can recommend additional skin treatments. For dry and lacklustre skin, peels and exfoliation are very effective.  A microdermabrasion will exfoliate the top layer of the skin using tiny aluminium oxide crystals or special wands coated with fine diamond crystals. These treatment takes less than 30 minutes and does not have any downtime. A microdermabrasion can be immediately followed up with chemical peel (during the same visit).

Wrinkles: The 411

What are wrinkles?

Wrinkles are creases or lines in the facial skin. Over time and with age, as we lose fat, collagen, and skin elasticity, more wrinkles develop.

There are two main types of wrinkles:

  • Dynamic wrinkles: These wrinkles are caused by years of making facial expressions. Frowning and smiling uses facial muscles under the skin – as we age, the skin sinks into the folds around the muscles, causing wrinkles.
  • Static wrinkles: These wrinkles are visible no matter the facial expression. Dynamic wrinkles may eventually become static wrinkles over time.

What causes wrinkles?

Wrinkles are caused by aging skin. As we age, our skin becomes less elastic i.e it doesn’t bounce back from facial expressions as quickly. Skin with less elasticity is also looser, causing folds in between the eyebrows, across the forehand, around the eyes and mouth. The wrinkles are further exaggerated by a loss of fat and collagen in our face.

External factors can cause wrinkling to be more noticeable or occur earlier in a person’s life, such as:

  • Smoking
  • Sun damage
  • Repeated dehydration (not drinking enough water)
  • Medications & illnesses
  • Poor skin health
  • Dehydration

Who gets wrinkles?

What is Sun-Damage And How To Treat It

PRP Infusion

Collagen Volumizers

Microdermabrasion

Using very small biodegradable crystals directed at the skin, Microdermabrasion removes the superficial layer of the skin. This dead and dry layer is what causes you to look older and less fresh.
